In a Munich or Hamburg Arztpraxis, the assistant at the front desk is "Sie" while the doctor might slip into "du" — mixing them inside one visit reads as either cold or inappropriate, both of which derail the visit. The noun-case shift between "Ich habe Schmerzen" (accusative: copy is precise) and "Ich habe Kopfschmerzen" (also accusative) is a single noun away from being wrong.
